Introduction

            In
2018, chronic diseases such as HIV/AIDS, stroke, diabetes, heart diseases, and
cancer were among the main causes of death and disability in the U.S. (Mokdad
et al., 2018).  The U.S.,  incurs more than $3.3 trillion in annual
health care costs.  Risky behaviors such
as poor nutrition, tobacco uses, and excessive use of alcohol and lack of
physical activity are known to cause many chronic diseases (Mokdad et al.,
2018).  Many governmental and
non-governmental healthcare organizations have focused their resources on
controlling and preventing chronic disease among community members through
population health management strategies. 
In response to the prevalence of chronic diseases, Minority AIDS Project
(MAP) has created a population health improvement plan based on population
health management strategies to improve one pervasive health concern in the
community—HIV/AIDS. HIV/AIDS in African American (AM) communities is the focus
plan.  The initiatives implemented under
the health improvement plan will use the best available evidence in Los Angeles
California communities gained through the evaluation of epidemiological,
demographic and environmental data. Additionally, the plan will apply
strategies for communicating health care improvement goals for African American
communities and health care professionals in an ethical, culturally sensitive,
and inclusive way.

 Environmental and Epidemiological Data about
African American communities

            Approximately
one million people in the U.S.,  today
are living with HIV. About 1 in 7 people do not know that they are infected
with HIV. In 2016, it was estimated that around 30, 000 Americans were infected
with HIV (Hess, 2017). Bisexuals and gay have a higher risk of getting infected
with HIV. Between 2010 and 2016,  HIV
infection among blacks and African Americans increased annually (Hess, 2017).
The population of Los Angeles  is estimated
to be 4 million people. These people are from diversity of culture and racial
backgrounds (“Resource: HIV/AIDS Statistics”, 2019). Since the first
HIV case was reported 31 years ago, about 30,000 people have tested positive
with HIV, with 13,000 still living (“Resource: HIV/AIDS Statistics”,
2019)

            Behavioral
risk factors such as transgender persons and gay are elevated risks for HIV
infection in Los Angeles (Holloway et al., 2012). Networks of African American
bisexual, gay and transgender networks such as house and ball communities who
compete in modeling and dance are the main target for HIV prevention by
governmental and non-governmental organizations.
