Head to Toe Assessment

A 44-year-old obese female is admitted to the emergency room with fever and severe abdominal pain on the upper right side that radiates to the scapula. She states that she feels nauseated. She states that the pain began at a friend’s house during a party and is becoming more severe. You can see that she is quite uncomfortable.

Perform a complete head-to-toe assessment and the analysis should include the following:
1. Topical headings to delineate systems.
2. For any system for which you do not have equipment, explain how you would do the assessment.
3. Detailed review of each system with normal and abnormal findings, along with normal laboratory findings for client age.
4. An analysis of age-specific risk reduction, health screen, and immunizations.
5. Your expectation of normal findings and what might indicate abnormal findings in your review of systems.
6. The differential diagnosis (disease) associated with possible abnormal findings.
7. A plan of care, must include nursing diagnosis, interventions, evaluation
8. Client and age-appropriate evidenced based practice strategies for health promotion.
9. Pharmacological treatments that can be used to address health issues for this client.
