Clinical Scenarios
Mr Franklin is a frail, thin-looking 85 year old man admitted to hospital with a 48 hour history of viral gastroenteritis. Mr Franklin usually lives independently with his wife of 60 years in their Brisbane home. He uses a walking stick to aid his balance. He has experienced severe vomiting and diarrhoea at home and is significantly dehydrated. He looks pale and says he still feels really nauseous and faint. His lips are dry and his oral mucus membranes look dull. He has a headache. His abdominal skin turgor is slack returning to form in 2 to 3 seconds. He hasn’t vomited for a few hours and has had 2 episodes of diarrhoea and abdominal cramps today. Mr Franklin is usually continent and is able to get up to the toilet himself. He has intravenous therapy 4% dextrose / 1/5th0.9% sodium chloride running for rehydration. His most recent vital signs are BP 110/65 mmhg lying down, pulse 96 beats/minute and regular, respirations 26 breathes/ minute, temperature 37.8C and Sa0295%. When the nurse inquires if he has pain Mr Franklin says he has a dull ache in his back and abdomen that has got a bit better since another nurse got him a hot pack.
Eloise is a 4 year old girl accompanied by her mother and admitted to hospital with a 48 hour history of viral gastroenteritis. Eloise is the youngest of 3 siblings in this family. She has experienced severe vomiting and diarrhoea at home and is significantly dehydrated. Her lips are dry and his oral mucus membranes look dull. Mum Jane says Eloise ‘is unusually quiet and wants to stay in bed or sitting on my lap which is very unusual for her’. Both Eloise and Jane are accommodated in a single room with Jane staying as a ‘boarding parent’. Eloise is usually able to use the toilet independently but since being unwell Jane has put her into nappies again due to the severity of the diarrhoea. She has intravenous therapy 4% dextrose / 1/5th 0.9% sodium chloride running for rehydration. Her abdominal skin turgor is a little slack returning to form in 1 to 2 seconds. Peripheral capillary refill on the skin of her knee is a little slow at 2 seconds. She hasn’t vomited for a few hours, has had 2 episodes of diarrhoea today and told Jane ‘my tummy hurts’.Her most recent vital signs are pulse 124 beats/minute and regular, respirations 26 breathes/ minute, temperature 37.8C, Sa02 98%. Jane is able to stay in hospital with Eloise as her other children are being cared for by their grandmother.
